I need help, calf muscle cramp.

I just came back to running after open heart surgery.
I have left running for many years, since I left the military.
I have been walking something between 5 - 10 km of walk plus jog daily. Problem is, the moment I started, in less than 500 meter, my calf and shin muscles will tensed up. I did do stretching before starting.
The pain is so bad at I have to walk. But I pressed on even when I felt the pain, by 4 km, the pan will subsided. But the moment I start to jog, the pain return.

Why is it so?

I just got a neutral probation shoe as recommended by the specialist. But it is not helping. How can I rid off the clamp? If that is the case, I cannot progress to running at all!

Do not recommend strenuous exercise, my grandfather had heart surgery, the doctor recommends: you can do some mild exercise, but if you feel uncomfortable immediately stop all activities, according to the situation appropriate to do some physical activity within daily reach , The amount of activity to not cause fatigue for the degree, began to walk and other gentle movements. The scope of activities should be indoor first outdoor.
